Secondary surgical interventions on previously operated nose

What is Secondary Rhinoplasty?

Secondary rhinoplasty is a term used to describe second correction surgeries performed on patients who have previously undergone nose aesthetics.

Simply put, it is a second or revision nose surgery performed on patients who are not satisfied with the result of the first surgery or who experience functional problems.

These operations not only correct aesthetic appearance; they also include functional goals such as eliminating breathing problems, restoring nasal support and re-establishing facial balance. Tertiary rhinoplasty refers to the third correction surgery performed on patients who have undergone two or more operations.

Why is a Separate Definition Needed?

Secondary or tertiary rhinoplasty operations require much more experience, patience and attention compared to primary rhinoplasty surgeries. The reasons for this are:

  • Tissue adhesions due to previous surgery make it difficult to clarify anatomical plans.
  • Since the nasal skin has thinned in the previous surgery, it can make irregularities in the underlying cartilage or bone more noticeable.
  • Collapse or loss of support in the nasal bridge may occur after excessive cartilage removal.
  • Hole (perforation) may occur in the internal nasal tissue due to mucosal and cartilage damage.
  • Since the previous surgery was performed by a different surgeon, the techniques and graft information used may not be fully known.
  • Therefore, the surgery time is longer and surgical planning must be more detailed.

Techniques Used in Secondary Rhinoplasty Surgery

The aim in these surgeries is to restore structure and support to the nose. In most cases, it is necessary to take grafts from another part of the body to replace cartilage or bone tissues that were removed in the previous surgery.

Ear Cartilage

(Conchal cartilage): Ideal for nasal tip support due to its flexible structure.

Rib Cartilage

(Costal cartilage): Provides strong support in advanced deformities.

Fascia

(Muscle sheath): Used to soften superficial irregularities and provide natural contour.

Op. Dr. Mahmut Ulvi Kayalı states that approximately 90% of secondary rhinoplasty cases require graft need in his clinical experiences.

Thanks to these grafts, collapses in the nasal structure are eliminated, nasal support is restored and natural aesthetic lines are created.

Surgery Process

  • Secondary rhinoplasty is usually performed with open technique.
  • This method provides the surgeon with a wider field of view and allows careful separation of adhesions left by the previous surgery.
  • Surgery time varies between 3 to 5 hours.
  • Cast and tapes are placed on the nose at the end of the surgery.
  • If comprehensive correction has been made in the internal nasal structure, silicone tampons can be used; otherwise, tampons are usually not placed.

Postoperative Period

The postoperative process is similar to primary rhinoplasty, but the healing process may be slightly longer due to previously operated tissues.

  • Edema and bruising may occur in the first 24 hours.
  • Cold application and keeping the head elevated support the reduction of edema.
  • Cast and tapes are removed in the 1st week.
  • The patient can return to daily activities from the 2nd week onwards.
  • The final nose shape completely settles within 12-18 months.

Conclusion

Secondary and tertiary rhinoplasty are one of the most challenging but most satisfying surgical procedures in the field of nose aesthetics. The aim is not only an aesthetic appearance but also functional and psychological recovery.

Plastic, Reconstructive and Aesthetic Surgeon Op. Dr. Mahmut Ulvi Kayalı performs secondary and tertiary rhinoplasty surgeries in his clinic in Antalya in accordance with modern surgical principles and with personalized planning.
